An app to help young people with rheumatic heart disease transition from care as a minor (under 18) to managing their condition as an adult.
We co-designed an app with patients, health professionals, a designer and developer to provide injection reminders, give the patient a visual calendar view of their level of protection and provide ability and reminders to book their next appointment.
